Hospice RN PositionGood Shepherd Hospice is seeking a compassionate and experienced RN to join our team serving patients and their families in Wichita and surrounding communities. The Hospice RN plans, implements and evaluates the nursing care of Hospice patients, working with families, caregivers and other members of the interdisciplinary team.Hospice Experience RequiredEvening Shift 2pm-10pmResponsibilitiesCoordinates Referrals and Communication Answers questions regarding admission criteria, hospice services and referral requestsTakes admissions referral and records information accurately on the referral forms and tracking sheetContacts physician's office for admission approval when appropriateContacts patient/family and makes appointment for admission interviewProcesses and Completes Admission Paperwork Provides accurate and appropriate documentation of patient/family servicesObtains written consent from patient, family or patient assignee, when criteria for hospice admission is metCommunicates with referral sources about appointments, MD orders, time frames for admission and any obstacles to an immediate admissionContacts patient/family within 24 hours to set-up appointment explaining hospice servicesWorks with Marketing Team Strong patient, family, referrer and facility satisfactionAppropriate market penetrationMaximization of hospice census goalsQualificationsHospice Experience Preferred (or previous experience in home health, geriatrics or general medical/surgical nursing required)Registered Nurses licenseCommitment to Providing Excellent Customer Service to Patients and FamiliesValid Driver's LicenseCan Successfully Pass Background Checks
