Nurse Practitioner for the Mobile Integrated Health Unit

Base Pay $105,000.00 / Year Employee Type Full-Time Exempt Required Degree Masters Contact information Name Jarquise Stowers Email View email address on click.appcast.io Description Ride with members of the Olathe Fire Department's Mobile Integrated Healthcare (MIH) team approximately three-and-a-half (3.5) days per week to provide primary care services to patients in their homes and other community-based settings. Provide primary care services in the clinic approximately one-half (0.5) day per week. Conduct comprehensive in-home assessments, including medical, functional, environmental, behavioral, and social determinants of health, to identify barriers to care and develop individualized treatment plans. Provide evaluation, diagnosis, treatment, and management of acute and chronic medical conditions within the Nurse Practitioner's scope of practice. Perform physical examinations, order and interpret diagnostic tests, prescribe medications, and perform procedures within the Nurse Practitioner's scope of practice. Deliver preventive care services, chronic disease management, medication reconciliation, health screenings, immunizations, and patient and caregiver education to promote improved health outcomes and reduce avoidable emergency department visits and hospitalizations. Collaborate closely with the Olathe Fire Department MIH team, Health Partnership Clinic providers, care managers, specialists, hospitals, and community partners to coordinate patient-centered care and ensure timely follow-up. Coordinate care transitions following emergency department visits or hospital discharges to improve continuity of care and reduce readmissions. Document patient encounters accurately and timely in the electronic health record and participate in care planning, patient tracking, quality improvement initiatives, and program reporting. Participate in Saturday clinic on a rotating basis, approximately four (4) times per year. Participate in shared call responsibilities with other Health Partnership Clinic medical providers. Maintain current clinical knowledge through continuing education and professional development. Perform other duties as assigned by the Chief Health Officer and/or Chief Executive Officer. Maintain regular attendance and punctuality as an essential function of the position. Qualifications Must be certified as a Family Medicine Nurse Practitioner. Must possess an active KS license as a Nurse Practitioner and maintain an active and unrestricted KS DEA number. Must be eligible for liability coverage via FTCA and private malpractice insurance carrier. Two years of experience within a community health center or similar environment preferred. Staff membership and privileges granted by the HPC Board of Directors (approval of credentialing and privileging applications). Ability to relate well with patients and other health care personnel. Social and cultural sensitivity appropriate to ethnically and economically diverse patient populations and employee base. Willingness to educate patients and the community in health maintenance and the role of the health care team. Ability to work under pressure. Commitment to expanding access to care for the underserved. Typical Physical Demands Prolonged sitting or standing may be required. Ability to lift to 25 pounds; occasionally up to 60 pounds. Those physical movements and the degree of mobility, manual dexterity and hand-eye coordination normally associated with normal medical work will be performed on a repetitive basis. The ability to distinguish letters and symbols as well as the ability to utilize telephones, computer terminals and copiers is required. Work under stressful conditions as well as irregular hours may be required. Frequent exposure to communicable disease, body fluids, toxic substances, medicinal preparations, radiation, and other conditions common to a clinical environment may routinely be encountered. Summary The Nurse Practitioner provides comprehensive, patient-centered health care services as a member of an interdisciplinary care team that includes physicians, advanced practice providers, nurses, behavioral health professionals, dental providers, and clinical support staff. This is a full-time position with a four-day, 10-hour-per-day work schedule. Health Partnership Clinic (HPC) utilizes a team-based approach to delivering high-quality, coordinated care.
