Housing Stability Case Manager
Care-Resource-Community-Health-Centers,-Inc.
Care Resource Community Health Centers, Inc. is seeking a HOPWA and Housing Opportunities Coordinator to screen and assess new clients, complete program applications, determine eligibility, and refer applicants to appropriate housing resources. You will manage caseloads, document progress, coordinate with internal and community partners, and support billing and reporting.
