Community Health Nurse: Home Visiting for Families
Parkland Health
Parkland Health seeks a Nurse Home Visitor (RN) to provide high-quality nursing care to patients in their homes. The RN coordinates care using the Parkland model, assesses needs, develops a patient-centered plan, and evaluates outcomes to prevent delays or complications. This role requires driving to patient homes and strong collaboration with the health care team. Required is a Bachelor's in Nursing, a current RN license in Texas, and a valid Texas driver license; BLS certification is also #J-18808-Ljbffr Parkland Health
