JOB SUMMARY The Intensive Care Manager (ICM) works alongside our medical staff and licensed behavioral health professionals to provide comprehensive case management services to patients seeking support with substance use. Services include referral and linkage, individualized care management, and support for adherence to available treatments, including Medication Assisted Treatment (MAT). The ICM practices within a Substance Use Prevention and Harm Reduction (SUPHR) framework using culturally sensitive, strength-based, and trauma- informed approaches. WHAT YOU WILL BE DOING Receive and manage patient referrals; collaborate with departmental staff to facilitate and expedite initial screening and intake. Provide ongoing support and care management to individuals engaged in substance use, utilizing a culturally sensitive, strength-based, trauma-informed approach to build and maintain a therapeutic alliance. Collaborate with patients and care teams to develop individualized care management plans that address identified needs and support progress toward patient-identified goals. Identify and connect patients to culturally appropriate resources across EHC and the broader community; provide referrals to outside agencies as appropriate. Monitor and evaluate each patient's recovery progression; communicate progress to the care team regularly and flag concerns as they arise. Address and amend care plans as needed in collaboration with patients and care teams, utilizing clinical assessment skills to match patients with the appropriate level of care, including referrals to higher levels of behavioral health care when indicated. Provide crisis intervention, crisis management, and safety planning as appropriate. Document all patient progress and services provided accurately and in a timely manner in the Electronic Medical Record (EMR).
