Requirements

• Required certification: Physical Therapist
• 1+ years of experience
• Key Requirements Summary: 1. Experience: Minimum 1 year of relevant, recent experience required; no first-time travelers accepted 2. Cases: Geriatric 3. Certifications: BLS required; Hospice/Home Health Checklist is mandatory 4. License: License in hand required at time of submission 5. Education: Not specified 6. EMR: HCHB (Homecare Homebase) proficiency required; OASIS experience required for staff completing SOC (Start of Care) 7. Shifts/Call: On-call required every 6th weekend; on-call rotation may be required - confirm during interview; Monday orientation 8. RTO: Must be submitted at the time of application 9. Returning Staff: Former permanent employees must be separated for a minimum of 6 months before returning as a traveler; must be approved by HR 10. Travel/Local: No locals accepted; territory covers Mossyrock and Morton area; traveler expected to be flexible with territory; active driver's license and personal transportation required; travel pairs accepted but each traveler assigned their own territory and must have own transportation 11.
