Complex Care Coordinator: Community Health Advocate

The Lake County Health Department and Community Health Center is seeking a Community Health Worker to support the Complex Care Management program within the Physical Health team. This role partners with an RN care manager and a complex care coordinator to identify, engage and support complex patients, improving outcomes and reducing unnecessary ED visits. Scheduled hours are 37.5 per week. The CHW will conduct outreach, build rapport with patients, develop care plans, track progress, and #J-18808-Ljbffr Lake County, IL
