Under the guidance of the medical director, the Primary Care Physician (PCP) delivers essential medical services to participants of the PACE Organization (PO). This role involves assessing patient needs, collaborating with the interdisciplinary team to create care plans, providing direct medical care, and evaluating the effectiveness of these plans. The PCP works closely with other healthcare professionals, making independent decisions regarding medical requirements and health management, and performs necessary medical procedures as required.

The Assisi House serves as the retirement convent for the Sisters of St. Francis of Philadelphia, situated directly across from the Our Lady of Angels Convent, the congregation's motherhouse. This facility adopts a person-centered care philosophy, fostering a familial atmosphere within its 125-bed, multi-level care environment, which includes specialized units for Alzheimer’s and dementia care.

Mercy Life Valley View offers both housing and the Mercy LIFE PACE program tailored for senior adults, specifically catering to a community of deaf seniors. Candidates must be open to learning American Sign Language.

This role entails working two 8-hour shifts at Assisi House and one 4-hour shift at Valley View, with flexible scheduling during the week. There is no requirement for night, weekend, on-call, or holiday coverage.

• Engage as a member of the interdisciplinary team (IDT) and participate in initial, semi-annual, unscheduled, and annual assessments. Report any changes in participants' baseline conditions to the relevant staff daily.
• Conduct initial and ongoing assessments of participants, develop care plans, and implement these plans. Ensure the primary care treatment plan aligns with the overall care strategy established by the interdisciplinary team.
• Refer participants to medical specialists as necessary. Oversee the care of participants in the nursing home, conducting regular visits in collaboration with the IDT, and maintain communication with nursing home staff for continuous monitoring of PACE participants on an individual basis. Admit nursing home participants to hospitals when required.
• Manage participants' interactions with medical specialists and inpatient care, ensuring effective communication and follow-up regarding visit outcomes.
• Analyze medical outcomes to identify best practices and enhance participant health. Complete necessary documentation and apply appropriate diagnostic coding.
• Advise the Medical Director on strategies to improve accountability of PO services to participants and referral sources, keeping the Medical Director informed about required resources as the program grows.
• Contribute to the development of policies, procedures, and standards of care; perform ongoing monitoring and evaluation of patient care practices and service delivery; and provide guidance and training to staff on medical and quality assurance matters.
• Participate in the provider on-call rotation as needed.

• Doctorate Degree (MD or DO) with a current and active plenary physician license to practice in the state of PO. CDS Certification and DEA Registration are mandatory.
• Current Basic Life Support (BLS) certification is required.
• Board Certification in Internal Medicine or Family Practice is essential; Board Certification in Geriatrics is preferred.
• Willingness to learn American Sign Language is necessary.
• A minimum of one (1) year of experience working with frail or elderly populations is required.
• A valid driver’s license, proof of insurance, and access to a personal vehicle are mandatory.
• Strong communication, problem-solving, and organizational skills are essential, with the ability to effectively engage with diverse individuals and groups.
• Proficiency in Microsoft Office Suite is required.
• Ability to adapt to changing priorities and demonstrate critical thinking skills while managing workload effectively.
• Periodic travel within the geographic region of the Center may be necessary.
