Develop and implement individualized treatment plans with measurable goals; monitor progress and adjust as needed. Deliver speech-language pathology services according to the plan of care; document treatment, progress notes, and discharge planning. Collaborate with an interdisciplinary team (nursing, rehabilitation, social work) to support resident goals and care plans. Provide education to residents and families on communication strategies and safe swallowing techniques; train facility staff as appropriate. Ensure accurate charting and compliance with regulatory and reimbursement requirements; maintain timely and complete documentation. Requirements Ability to work in a skilled nursing facility setting and participate in an acute care-oriented rehabilitation environment. Demonstrated ability to assess and treat speech, language, cognitive-communication, and swallowing disorders. Strong documentation skills and adherence to regulatory standards. Effective collaboration with multidisciplinary teams and support staff. Availability for a 5x8 shift (08:00–16:30) with guaranteed hours per week.
