Job Summary Jackson Therapy Partners is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a travel job in Sandwich, Illinois. Minimum Qualifications Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active Illinois Physical Therapy license required to start the assignment. BLS certification from the American Heart Association or American Red Cross may be required. Experience working in skilled nursing or long‑term care settings preferred. Ability to manage typical SNF caseloads and documentation requirements. Preferred Qualifications Prior travel therapy experience preferred. Experience with electronic medical records systems preferred. Advanced training in geriatric or post‑acute care preferred. Assignment Details Shift hours: 08:30–16:30. Schedule: five 8‑hour shifts per week. Position requires approximately 35 hours per week. Candidate may be required to float to other facility locations as needed.
