We are currently offering an opportunity for a full time Social Worker to support adult bone marrow transplant patients. Joining our team comes with several benefits, including: Free parking for regular status caregivers Full benefits and retirement Support for continuing education, career growth, and professional development A Brief Overview Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves as a resource within the social work department and the hospital. What You Will Do Provide psychosocial assessment / intervention to assist patients/families, including clinically complex cases. Provide age specific care to patients/families in all age groups. Develop, document and implement a comprehensive plan based on identified psychosocial needs and/or discharge needs. Develop plans with attention to individual patient and family goals. Provide linkage to community resources and UH network services to facilitate optimal care, and serve as liaison to community agencies. Communicate and collaborate with the work team using a multidisciplinary approach. Provide guidance and serve as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers. Assist with recruitment, orientation, mentoring, education of new staff and or social work interns. Additional Responsibilities Perform other duties as assigned. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace. Education Master's Degree from an accredited school of Social Work (Required) Work Experience Hospital/Health Agency experience (Preferred) Knowledge, Skills, & Abilities Theoretical and clinical knowledge regarding health care delivery (Required proficiency) Multi‑tasks and prioritizes work (Required proficiency) Works autonomously (Required proficiency) Communicates effectively with persons of various backgrounds (Required proficiency) Licenses and Certifications Licensed Social Worker (LSW) (Required) Licensed Independent Social Worker (LISW) (Preferred within one year of eligibility) Preferred Qualifications / Position Details At least one year of experience is preferred. Previous work with oncology or chronic diseases is also preferred. This outpatient Social Worker will support bone marrow transplant teams within Seidman Cancer Center. Responsibilities will include performing psychosocial assessments for patients considering transplants, supporting patients with emotional and mental health issues, providing various social services resources, etc. There is an on-call rotation (about every 9 weeks).
