Requirements:

• Graduate of an accredited Occupational Therapy program.
• Current Occupational Therapist license in the state of practice.
• Prior skilled nursing facility or geriatric rehabilitation experience preferred.

Roles & Responsibilities:

• Evaluate residents' functional abilities, including activities of daily living (ADLs), mobility, cognition, and psychosocial factors.
• Develop and implement individualized treatment plans based on evaluation findings and physician orders.
• Provide direct occupational therapy services in accordance with state and federal regulations and facility policies.
• Monitor and document resident progress, adjusting treatment plans as necessary to meet goals.
• Participate in care planning meetings and collaborate with the interdisciplinary team.
• Educate residents, families, and caregivers on strategies to promote safety and independence.
