Clinical Partner/Medical Assistant

HarmonyCares is seeking a Clinical Partner/Medical Assistant to support our home-based primary care and palliative care teams, Monday through Friday. In this role, you will assist providers with patient assessments, obtain vital signs, perform basic tests, and document care in the EMR. You’ll help coordinate home and telehealth visits, prepare patients and equipment, and reinforce care instructions for medically complex, home-limited patients. Working within an interdisciplinary, mission-driven team, you’ll play a key role in improving access, reducing avoidable hospitalizations, and delivering compassionate, patient-first care.
