RN Case Manager - Home Health

The RN Case Manager provides skilled nursing, care, and case management for assigned patients in their place of residence. This role involves admitting patients onto services, coordinating and integrating services to ensure continuity of care, and facilitating the achievement of quality, clinical, and cost outcomes. The RN Case Manager will negotiate, procure, and coordinate services and resources needed by the patient or family, utilizing the nursing process of assessment, planning, implementation of care, and evaluation through an inter-disciplinary team approach.

Responsibilities

• Provides skilled nursing and coordinates care to patients and family members in a residential setting.
• Observes, assesses, evaluates, and plans interventions based on patients' physical and psychosocial statuses.
• Oversees care provided by Home Health Aides and CNAs and evaluates compliance with the plan of care.
• Ensures continuity of care through effective communication and case conferences with other disciplines and agencies.
• Delivers quality care and symptom management to promote optimal health and quality of life.
• Maintains accurate and complete patient records in accordance with agency standards and OASIS requirements.
• Participates in team scheduling coordination and on-call rotation as necessary to meet patient care needs.
• Engages in quality improvement activities and attends in-service training as assigned.

Schedule

Typical schedule is Monday to Friday, 8:00 AM to 5:00 PM, with on-call and weekend rotation.

Required Experience / Certifications / Licensure

• Current RN license in the State of Montana (BSN preferred).
• Valid driver's license, appropriate auto insurance, and reliable transportation required.
• 2 years of recent acute care, home care, or hospice experience preferred, including skills like venipuncture, catheterization, wound care, and/or parenteral therapy.
• Experience with HCHB strongly preferred.
