Job Summary

Responsibilities

• Provide direct patient care to a diverse range of residents, including short-term rehabilitation patients and long-term care residents

• Assess patient needs, develop individualized care plans, and administer medications as prescribed by physicians

• Collaborate with multidisciplinary teams, including physicians, therapists, and other healthcare professionals to ensure seamless patient care

• Monitor patient progress, identify changes in condition, and communicate effectively with patients and families

• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Requirements

• Current RN licensure in the state of Pennsylvania

• Bachelor's degree in Nursing or related field

• 1+ years of experience as a Registered Nurse
