Sandy Lake Community Health Center: 5344 Franklin Street, Sandy Lake, PA, 16145

Sheakleyville Health Center: 3339 Perry Highway, Sheakleyville, PA 16151

• Monday 8:00 am - 5:00 pm
• Tuesday 8:00 am - 7:00 pm
• Wednesday 8:30 am - 5:00 pm
• Thursday 8:30 am - 5:00 pm
• Friday 8:30 am - 3:00 pm

Qualifications

• Graduate from an accredited School for Nurse Practitioners.
• Certified as a Registered Family Nurse Practitioner
• Current PA License; and Current DEA License
• Knowledge of Electronic Health Records
• Ability to provide quality, comprehensive, continuity of medical care in a compassionate manner with minimum supervision.
