Growing together. Care at Home is a collaborative, population health-focused care delivery model serving Dual Eligible Special Needs Plan (DSNP) members-individuals with significant medical, behavioral, and social complexity, including serious mental illness (SMI) and substance use disorders (SUD). In this role, you will partner with an interdisciplinary care team to deliver high-quality, evidence-based behavioral health care. As a Behavioral Health APC (PMHNP), you will play a critical role within an interdisciplinary care team, bringing psychiatric expertise into a home-based and population health model of care. You will support both direct patient care and consultative care. If you live in Wisconsin or Iowa and the have licensure, you will have the flexibility to work remotely* as you take on some tough challenges. Primary Responsibilities: Conduct telehealth-based psychiatric evaluations for patients with serious mental illness and substance use conditionsDiagnose, treat, and prescribe evidence-based pharmacologic and non-pharmacologic interventionsProvide bridge stabilization through medication management and follow-up care for high-risk patientsWork with interdisciplinary care team to ensure connection to community mental health services.Act as a behavioral health consultant to an interdisciplinary care team including providers, RNs, and social workersSupport management of depression, anxiety, SMI, and SUDProvide real-time and asynchronous consultation, including treatment recommendations and care planningHelp triage patients to appropriate levels of care, including specialty mental health and community-based resourcesSupport care continuity by partnering with external behavioral health providers and systems of careFacilitate engagement in specialty care when neededDeliver behavioral health education to interdisciplinary teams managing medically complex patientsParticipate in case conferences, care rounds, and team-based decision-makingContribute to a culture of continuous quality improvement and clinical excellenceDocument all patient encounters, consults, and care coordination efforts in the EMRCommunicate clearly and effectively across care teams and community partners

3+ years of post-certification clinical experienceExperience in consultation-liaison psychiatry, population health, or integrated care modelsExperience with geriatric, DSNP, or medically complex populationsTelepsychiatry experience *All employees working remotely will be required to adhere to UnitedHealth Group's Telecommuter Policy Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
