**About CAN-AM Sports Medicine**

We're a cash-based sports medicine and performance rehab clinic in Phoenix serving active adults 40+ — golfers, pickleball players, lifters, runners, and weekend athletes who've tried conventional PT or chiropractic and didn't get the results they were promised. We don't do band-aid care. We solve root-cause problems with movement-based rehab, strength integration, and 1-on-1 attention.

• **Clinical autonomy within a performance-driven system** — you choose the techniques, treatment selection, and progression. We measure the outcomes. No insurance dictating your care, no predetermined protocols — but you own your results.
• A defined patient journey: Discovery Visit → Evaluation → Plan of Care, with documented systems assessment framework, structured SOAP notes, conversion tracking) so you know exactly how care progresses
• Mentorship directly from the owner/lead clinician on both clinical skill and patient conversion

**What we're looking for**

• Licensed Doctor of Chiropractic in Arizona
• Licensed Physical Therapist in Arizona 
• CCSP, CSCS, or MS in a related field strongly preferred
• Demonstrated experience in sports rehab, performance training, or strength and conditioning — not just adjustments
• Comfortable communicating value and converting evaluations into plans of care (we will train and coach this — but you need to be willing to own it)
• Track record of holding patients accountable to outcomes, not just symptom relief
• Self-directed, professional, and confident in your clinical judgment

**Responsibilities**

• Deliver high-quality, individualized rehab to active adults and athletes
• Run comprehensive movement assessments and build progressive rehab plans
• Own your patient conversion: Eval → POC → 2nd POC → long-term retention
• Track and improve your own conversion and retention metrics (we measure this)
• Support clinic growth through workshops, gym partnerships, and community events
• Educate patients on long-term performance, not just pain relief

**Compensation**

• **Base salary: $70,000 - $85,000
• **Performance bonuses** tied to clearly defined conversion and retention metrics

**Schedule**

Monday–Friday up to 40 hours a week, with occasional weekend events (workshops, community outreach)

**Performance expectations — read carefully**

We run on metrics. Within your first 90 days, you'll be evaluated on specific conversion benchmarks (Eval-to-POC %, retention, revenue per patient, patient outcomes). We give you the system, the training, and the mentorship to hit them. In exchange, we expect you to hit them. If that sounds like accountability you'd welcome, apply.
