Care Coordinator
Location: East Broadway
Schedule: Monday–Friday, 8:00 AM – 4:30 PM
Pay: $17.50/hour
Contract: Long-term temp (minimum 1 year)
About the Role:
Family Health Centers (FHC) is seeking a dedicated and compassionate Care Coordinator to join our Population Health team, focusing on patients with Passport Molina insurance. This role is vital in supporting our mission to provide high-quality care by connecting patients with preventive services and closing care gaps.
Key Responsibilities:
- Schedule patient appointments and follow-ups
- Monitor and complete preventive health measures
- Alert providers to diagnoses and care gaps for risk adjustment
- Use registries to inform care teams of needed preventive services
- Counsel patients on recommended health screenings
- Ensure accurate data entry into electronic health records (EHR)
- Identify and support patients with language or communication barriers
- Scan and upload documents into EHR
- Outreach to patients over-utilizing emergency services and connect them with primary care
Ideal Candidate:
- Organized and able to prioritize tasks independently
- Strong communication and interpersonal skills
- Passionate about improving patient outcomes
- Experience in healthcare or care coordination preferred
