Home Care Registered Nurse (RN)

University Home Care Inc is seeking a compassionate and skilled Registered Nurse (RN) to join our team as a Home Care RN. In this role, you will provide personalized nursing care to patients in their homes, ensuring their health, comfort, and well-being. You will work closely with patients, families, and healthcare providers to develop and implement individualized care plans, monitor patient progress, and offer education and support to promote recovery and independence.

Responsibilities

• Provide skilled nursing care to patients in their homes according to physician orders and care plans.
• Perform comprehensive patient assessments to identify healthcare needs and changes in condition.
• Administer medications, treatments, and injections as prescribed.
• Collaborate with interdisciplinary team members to ensure coordinated and consistent care.
• Educate patients and families on disease management, medication administration, and health promotion.
• Maintain accurate and timely documentation of patient care, observations, and outcomes.
• Ensure adherence to infection control and safety protocols.
• Report any changes in patient condition to physicians and other healthcare professionals promptly.

Requirements

• Current and valid RN license in the state of Michigan.
• Previous experience in home health nursing preferred.
• Ability to work independently and manage a patient caseload efficiently.
• Strong communication and interpersonal skills.
• Proficient documentation and clinical assessment skills.
• Reliable transportation and ability to travel to patient homes within the Southeast Mi Region.
• CPR certification and current healthcare provider CPR card.
