Licensed Practical Nurse (LPN) - Home Health

DPS Home Health Agency Inc. is seeking caring, dependable Licensed Practical Nurses (LPNs) to provide skilled nursing care to patients in their homes. We offer flexible scheduling, supportive clinical leadership, and opportunities for professional growth.

About the Role

As a Home Health LPN, you will provide skilled nursing care under the direction of a Registered Nurse (RN) and the patient’s physician. You will carry out treatments, monitor patient condition, assist with care plan implementation, and support patients and caregivers through education and hands-on care.

Qualifications

• Active Florida Licensed Practical Nurse (LPN) license.

• Current CPR/BLS certification.

• Excellent assessment, communication, and organizational skills.

• Valid Florida driver’s license and reliable transportation.

• Ability to work independently while providing compassionate, patient-centered care.

Preferred Experience

At least one year of nursing experience is preferred. Home health experience is an asset but is not required.

Preference will be given to candidates with experience in one or more of the following:

• Home health.

• Wound care.

• IV therapy.

• Chronic disease management.

• Diabetes education.

• Tracheostomy care.

• Gastrostomy tube (G-tube) care.

Responsibilities

• Provide skilled nursing care in accordance with the plan of care established by the RN and physician.

• Administer medications and treatments as ordered.

• Monitor patient condition and report changes to the supervising RN.

• Provide wound care and other skilled nursing interventions.

• Assist with patient and family education on disease management, medications, and home care.

• Maintain accurate and timely clinical documentation.

• Coordinate care with the RN case manager, physicians, and other members of the healthcare team.

• Ensure compliance with Medicare, state, and company policies.
