* Competitive wages (not pay-per-visit)* Mileage reimbursement* Professional development & continuing education support* Trusted, nationally recognized healthcare system* Supportive team culture focused on growth, collaboration, and innovation* A work environment that values your expertise, autonomy, and well-being* Provide high-quality, evidence-based physical therapy in patients’ homes* Deliver care according to individualized treatment plans developed by supervising PTs* Promote patient safety, mobility, and functional independence* Educate patients and caregivers on exercises, mobility strategies, and home safety* Accurately document clinical notes and patient progress in EMR systems* Communicate effectively with interdisciplinary team members* Travel independently to patient homes within designated service areas* Active Physical Therapist (PT) license in the state of Tennessee* Minimum 1 year of PT experience* Strong communication and assessment skills* Ability to work independently with a flexible caseload* A valid driver’s license, reliable transportation, and ability to maintain a satisfactory driving record* Home Health experience* Experience with EMR documentation systems* Strong understanding of rehabilitation principles and functional mobility* Formulate functional treatment plan that is goal focused, individualized, safe and age-specific to address deficits and patient/caregiver areas of concern.* Evaluate patient/caregiver responses and modify plan of care according to VHCS and VUMC standards.* Administer and score standardized and criterion referenced tests.* Interpret the results of the assessment and their significance to the patient, family, referring physician and other members of the health care team.* Evaluate and incorporate new research findings into the assessment and treatment of patients.* Assist with establishing or modifying existing "Standards of Practice" for specific patient groups.* Complete documentation according to agency policy* Meet productivity standards.* Demonstrate accountability as an individual and team member in meeting patient/family needs.* Participate in identifying cost effective measures for clinical care.* Manage clinical assignment in an organized, efficient manner using human and other resources.* Continuum of Care/Provision of Care* Provide therapeutic interventions for identified objectives.* Evaluate patient/caregiver responses and modify plan of care according to VHCS and VUMC standards.* Assure coordination of care for all patients including multi-disciplinary coordination as a case manager.* Identify and recommend for referral to other services as indicated.* Assure patient safety.* Establish relationship of trust and confidentiality with patient/caregivers.* Practice Infection Control.* Meet obligation for reimbursement of the services.* Patient/Caregiver, Team & Community Education* Incorporate new information from ongoing dialogue with patient, family, team, physician and payer into treatment plan* Provide education/consultation to patient/caregiver/team/community.* Assess patient/caregiver learning needs, coordinate necessary teaching, and evaluate teaching outcomes.* Effectively identify and use appropriate educational tools and materials.* Plan educational programs to assist team/department with identified need.* Develop and achieve personal and professional goals.* Participate in quality improvement activities.* Identify need for professional growth and seeks appropriate professional development opportunities, obtaining a minimal number of educational activities annually that meet licensure requirements.* Team/ Organization:* Function as an integral member of the team, demonstrating ability to: respect roles and professional boundaries of self and others; follow through with assigned roles and required activities; understand/comply with policies and procedures.* Precept new staff.* Attend/participate in agency and committee meetings, including agency and discipline-specific Unit Boards, utilizing the Shared Governance model.* Adhere to and promote VHCS Mission and Vision.* Integrate professional practice/job responsibilities and organizational expectations.* Comply with standards, policies/procedures, laws and organizational expectations.* Strive to meet organizational service, satisfaction and cost goals.* Represent VHCS on committees, etc. outside the agency, as applicable.## PEOPLE ARE AT THE HEART OF ALL WE DO.***Our vision:****The world leader in advancing personalized health.****Our mission:****personalizing the patient experience through our caring spirit and distinctive capabilities.****Making Health Care Personal.***We’re looking for like-minded individuals driven to make a difference.
