Position Overview
The Social Worker is responsible for assessing the mental and emotional functioning of individuals and families residing in shelter-based or supportive housing programs. This role provides direct clinical services, crisis intervention, and referrals as needed, and offers clinical supervision to Case Managers through case analysis, service planning, and ongoing guidance. The Social Worker plays a critical role in ensuring trauma-informed, culturally responsive care while supporting clients in achieving stability and long-term goals.



Major Duties and Responsibilities

• Assess and evaluate new, existing, and prospective individuals and families referred by the NYC Department of Homeless Services (Client).
• Complete comprehensive clinical and psychosocial evaluations upon admission.
• Develop and monitor Individual Living Plans (ILPs) for families, including all household members.
• Provide therapeutic support and short-term clinical interventions as needed.
• Provide clinical supervision to Case Managers, including case review, treatment planning, and documentation oversight.
• Conduct clinical case conferences with clients in collaboration with internal staff and external service providers.
• Participate in ongoing caseload conferences, clinical meetings, and interdisciplinary team discussions.
• Respond to and manage crisis interventions, including on-call coverage as required.
• Coordinate mental health and psychiatric care, including referrals to external providers.
• Develop and maintain a network of social service and mental health resources.
• Facilitate and lead clinical or psychoeducational group sessions.
• Monitor resident records to ensure compliance with best practices and regulatory standards.
• Function independently during facility coverage, taking appropriate clinical action during emergencies or crisis situations.
• Regularly assess client safety, stability, and overall well-being.
• Participate in individual and group clinical supervision with a Social Work Supervisor.
• Attend trainings, seminars, and external meetings related to service delivery and professional development.
• Assist with program reporting, documentation review, and outcome evaluation.
• Review progress notes and service plans to ensure clinical quality and goal alignment.
• Monitor and support client compliance with service plans.
• Use trauma-informed strategies to engage individuals with complex trauma histories.
• Perform other duties as assigned by the Social Work Supervisor or program leadership.

Education Requirements

• Master's degree in Social Work or a related clinical mental health field required
• Degree must include clinical training
• Professional licensure is preferred not required

Required Skills and Experience

• Minimum of three (3) years of experience working with homeless populations.
• Demonstrated experience working with disenfranchised communities, including:
  • LGBTQ individuals
  • Survivors of trauma
  • Older adults
  • Individuals with substance use and/or mental health challenges
• Clinical experience conducting psychosocial assessments and developing individualized service or treatment plans.
• Strong knowledge of trauma-informed and culturally responsive care.
• Familiarity with NYC Client regulations and reporting systems preferred.
• Excellent verbal and written communication skills, including clinical documentation.
• Experience facilitating group counseling or therapeutic groups.
• Ability to work collaboratively with multidisciplinary teams, external agencies, and community partners.
• Proficiency with Microsoft Office and electronic record systems.
• Bilingual Spanish or Asian Dialects Preferred.
