Key Responsibilities

Evaluate and treat patients in a skilled nursing facility setting to restore function and improve quality of life
Develop and implement individualized physical therapy treatment plans based on patient needs and goals
Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
Educate patients and caregivers on therapeutic exercises and home safety strategies

Required Qualifications

Active Physical Therapist (PT) license in the state of Massachusetts
Graduate of an accredited Physical Therapy program
Prior experience working in a skilled nursing or long-term care environment
Ability to work day shift hours

Preferred Experience / Skills

Experience with MDS assessments and Medicare documentation requirements
Familiarity with electronic medical records (EMR) systems
Strong communication and patient-centered care skills
