Now Hiring: Care Delivery Manager (RN) Managed Long-Term Care (MLTC)

Location: New York, NY

Pay: $67.00 - $70.00/hour

Schedule: Monday-Friday | 8:30 AM - 5:00 PM (1-hour lunch)

Opportunity: Long-Term Contract | Temp-to-Perm

Are you an experienced Registered Nurse (RN) with Managed Long-Term Care (MLTC) leadership experience? Join a mission-driven healthcare organization where you'll lead a dedicated care management team and make a meaningful impact on the lives of frail and elderly members throughout New York.

What You'll Do:

• Lead and oversee individualized care planning for MLTC members
• Facilitate Interdisciplinary Team (IDT) meetings and Service Delivery Requests (SDRs)
• Supervise, mentor, and support Care Management staff
• Review clinical documentation, assessments, and care plans for quality and compliance
• Coordinate care transitions between hospitals, rehab facilities, nursing homes, home care, and community settings
• Collaborate with clinicians to ensure accurate UAS assessments and comprehensive care planning
• Investigate incidents, grievances, and complaints while ensuring timely resolution
• Monitor member outcomes and improve quality of care
• Participate in hiring, onboarding, coaching, and performance evaluations
• Ensure compliance with all NYS, CMS, MLTC, and PACE regulations
• Serve as Director of Patient Services (DPS) for LHCSA when assigned

Qualifications:

• Graduate of an accredited Nursing Program (BSN preferred)
• Active, unrestricted New York State RN License
• NYSED Infection Control Certification
• Minimum 2+ years of nursing management/administrative experience in one of the following: CHHA, LHCSA, Acute Care, Medical-Surgical, Critical Care, Skilled Nursing, Related Healthcare Settings
• Minimum 1 year working with a frail and/or elderly population
• Prior supervisory experience required
• Strong leadership, communication, and customer service skills
• Managed Long-Term Care (MLTC) experience required
• Case Management Certification preferred within two years of hire
