As a Licensed Practical Nurse (LPN), you will provide direct nursing care to residents while collaborating with Registered Nurses, physicians, therapists, and other healthcare professionals. Your clinical expertise and compassionate approach help ensure residents receive exceptional care every shift.

Key responsibilities include administering medications and treatments as prescribed, monitoring, assessing, and documenting residents' health status, reporting changes in condition to the supervising RN or physician, assisting residents with activities of daily living (ADLs) as needed, collaborating with the rehabilitation team to support recovery goals, maintaining accurate documentation in accordance with facility policies and state/federal regulations, providing education and emotional support to residents and families, and following infection control and safety protocols to maintain a safe environment.

Qualifications include a current and valid Licensed Practical Nurse (LPN) license, strong clinical skills, including medication administration and wound care, excellent communication and interpersonal skills, ability to work effectively in a team-oriented environment, compassionate, patient-focused approach to care, and BLS/CPR certification required.
