RN Case Manager

RN Case Manager Hybrid Role | 2 Days Remote / 3 Days In the Field or Office Monday–Friday, 8:00 AM–4:00 PM No nights, weekends, or on-call Looking for a nursing role with better balance and more flexibility? We're seeking an experienced RN Case Manager to support seniors with complex health needs in a community-based setting. You'll be working closely with patients in their homes and coordinating care with providers and care teams to help seniors stay safe, supported, and independent at home. This is a hybrid role with two remote days each week, plus field and office work the rest of the time—so you get a nice mix of independence, patient interaction, and work-from-home flexibility.
