About the Role:
Newport Hospice is looking for a compassionate and dedicated On-Call LVN Runner to join our team in San Bernardino, CA. This is a meaningful opportunity to make a real difference in the lives of patients and families during their most critical moments, providing essential clinical support in a fast-paced, mission-driven environment.
- Respond to on-call patient needs, providing timely bedside nursing support in home and facility settings
- Perform skilled nursing assessments, symptom management, and comfort-focused interventions
- Administer medications and treatments as directed by the care plan and supervising RN
- Communicate patient status updates clearly to the on-call RN and interdisciplinary hospice team
- Provide emotional support and education to patients and families during end-of-life care
- Accurately document clinical observations and interventions in the patient medical record
- Coordinate with hospice staff to ensure seamless continuity of care during after-hours visits
- Active Licensed Vocational Nurse (LVN) license in the state of California
- Previous hospice, palliative care, or home health nursing experience preferred
- Ability to work flexible on-call shifts, including evenings, weekends, and holidays
- Strong clinical assessment and critical thinking skills in high-acuity situations
- Reliable transportation and valid California driver's license required
- Compassionate, patient-centered approach with excellent communication skills
- Proficiency in electronic medical records (EMR) documentation
