Arise Collective is a unique organization operating at the nexus of prison chaplaincy, womens issues, and social justice with a mission to equip women with the tools and support they need to heal, grow and thrive, both in prison and in the communities to which they return. The organization provides chaplaincy services and transition education inside womens prisons; comprehensive and holistic wraparound services for formerly incarcerated women as they reenter society, including housing and access to education and employment opportunities; and community engagement programs and presentations to educate the public about issues of incarceration and reentry. To learn more about the organization, please visit CURRENT NEED The Case Manager is a key role within the organization. The successful candidate will assist individuals transitioning from incarceration back into the community through holistic assessment and planning, resource referral and coordination, advocacy, monitoring and support, life skills and empowerment coaching, crisis intervention, and collaboration. The successful candidate is highly organized, able to balance collaborative with independent work, and skilled at operating in a fast-paced environment while remaining good-natured, resourceful, and efficient. A high level of maturity, integrity, professionalism, discretion, sound judgment and decision-making skills, initiative, tact, confidentiality, and self-care are crucial. In collaboration with a multidisciplinary team that includes both peer and clinical staff, responsibilities include: DUTIES AND RESPONSIBILITIESRESIDENTIAL CASE MANAGEMENT (40%) Develop personalized reentry plans addressing housing, employment, education, healthcare, transportation, substance use, mental health, family reunification, budgeting and finance, and other relevant needs based on comprehensive assessments. Provide ongoing support, guidance and encouragement to individuals as they navigate the reentry process. Provide individual support and mentorship to residents, fostering a sense of empowerment and accountability. Collaborate with other staff members, including program managers, peer support specialists and mental and behavioral health professionals. Monitor participants progress and adjust plans as needed. Maintain accurate and confidential case records. Manage discharge plans upon completion of the program. NON-RESIDENTIAL PROGRAMMING (35%) Connect individuals with community resources and services that support their successful reentry, including housing assistance, job training and placement services, healthcare providers, mental health counseling, substance use treatment, and legal assistance. Assist with the navigation of bureaucratic systems, access necessary services, and address barriers or challenges encountered along the way; advocate for fair treatment, equal opportunities, and the removal of obstacles to successful reentry. Conduct regular check-ins, monitor progress toward goals, and address setbacks or barriers. Offer emotional support and crisis intervention when needed. Offer education and coaching on essential life skills. Provide immediate support and assistance to navigate challenges and prevent further harm. Collaborate with other service providers, community organizations, government agencies, employers, and stakeholders involved in the reentry process. PROGRAM SUPPORT (10%) Document and maintain regular communication via call logs with past participants (alumni support) and potential participants (referral and waitlist management), providing program updates, resources, and status information. Build and maintain relationships with potential employers and landlords to expand opportunities for participant placement and successful reentry. Assist in communication and coordination with providers to ensure the ongoing needs of participants are met and monitored. Support the collection and analysis of program feedback and outcomes to inform program development and continuous improvement. Respond to inquiries related to program services, ensuring timely and accurate communication in accordance with the organizations communication log. Participate in the delivery and/or oversight of programming, where appropriate. Serve in the on-call rotation, responding to urgent participant or program matters as needed. Maintain accurate records related to program activities. ORGANIZATIONAL RESPONSIBILITIES (15%) Attend staff meetings, board meetings, and retreats. Participate in opportunities to educate and advocate for increased awareness of the issues surrounding womens incarceration and incarceration in general. Participate in case management meetings, networking events, and community outreach efforts to ensure a coordinated and holistic approach to reintegration. Participate in ongoing training and professional development opportunities. Other responsibilities and special projects, as needed. REQUIRED QUALIFICATIONS AND ATTRIBUTES 5-8 years of progressively responsible experience providing direct services to clients. Associate degree in social work, criminal justice or a related field, bachelors degree preferred. Inclusive, empathetic worldview and demonstrated skill working across lines of difference. Experience working with high-risk women and youth; experience with trauma-informed modalities a plus. Criminal justice education, background, or experience. Progressively responsible experience in case management and service coordination. Strong understanding of reentry challenges and best practices. High level of emotional intelligence and ability to bring maturity, integrity, sound judgment, professionalism, tact and self-care to staff and client interactions. Effective and culturally competent oral and written communication skills with excellent presentation and grant writing skills. Strong organizational abilities, including critical thinking, planning, delegating, program development, program evaluation, and management. Proficient in use of technology including standard operating systems and MS Office products as well as experienced setting up and using electronic health records, case management tracking systems, and other database platforms. Ability to work both independently and on a team within a high stress environment while maintaining policies, professional standards, and a balanced attitude. PREFERRED QUALIFICATIONS AND ATTRIBUTES: Lived experience in or significant personal understanding of the dynamics and factors contributing to the criminal legal, mental health, substance use and/or domestic violence system involvement. Social justice orientation with experience in effective organizing strategies. Comfort working within an inclusive faith-based environment. Residential treatment or group housing facility experience. Strong facilitation skills, including designing and leading professional trainings. Entrepreneurial spirit and the ability to be flexible and juggle multiple, disparate roles within a vibrant and growing organizational culture. Willingness to extend kindness, compassion and accountability to oneself and others. ADDITIONAL REQUIREMENTS Possess valid NC Drivers License, satisfactory driving record and proof of insurance. Be able to complete and pass NCDAC-required and/or organizational background check. PRIMARY RELATIONSHIPSThis position will work with the organizations program staff and will interact regularly with other staff and volunteers. Will have substantial contact with participants, their families and significant others, program partners, community treatment providers, community corrections (probation and parole), the faith community, and others in service provision. PHYSICAL REQUIREMENTSThis position requires ordinary ambulatory skills sufficient to visit other locations, as well as the ability to stand, walk, stoop, kneel, crouch, and occasionally manipulate (lift, carry, move) light to medium weights of up to 20 pounds. Requires good hand-eye-foot coordination; arm, hand and finger dexterity, including ability to grasp; visual acuity to use a keyboard, operate equipment, read information; use of auditory senses; and ability to perform basic math (addition, subtraction, multiplication, division, standard measurements). WORK REQUIREMENTSThis is both an on- and off-site position with a primary purpose to provide these services in the community to enhance participants ability to overcome obstacles and achieve their goals. The regular schedule is negotiable within a standard 40-hour work week, and includes a hybrid model of time in the office and remote work. Travel to clients locations is required, along with occasional but regular evenings and weekends. The impact of actions is great in this position with potential harm to participant safety and well-being. JOB TYPE: Salaried full-time exempt position. SALARY RANGE: $50,000 - $60,000 for 40-hours/week, depending on qualifications and experience.
