These platforms provide additional avenues for employees to give feedback about their work experience and share what is important to them.QualificationsRequired Skills:Doctor of Pharmacy from an ACPE accredited college of pharmacyAt least one year of hospital pharmacy experienceSupervisory experience preferred.Additional Qualifications/Skills:Strong analytical, problem-solving, written communication and interpersonal skillsMust be flexible with work scheduleDependable attendance and job performanceLicensed or license-eligible in the State of TexasResponsibilitiesEnsures safe transitions of care by completing comprehensive medication reconciliation and final verification of admission and discharge orders for all patients.Evaluate and verify WellSky physician's orders for therapeutic appropriateness, proper dosing, optimal dosage form, drug interactions, allergic reactions, route and rate of administration and therapeutic redundancyClarifies physicians' orders, when necessary, through direct and timely contact with prescriber or designated practitionerDemonstrates knowledge with antimicrobial stewardship, which includes therapy optimization, de-escalation, and selection of appropriate, evidence-based antimicrobial regimens.Assists with monitoring and appropriate prescribing of anticoagulation therapyProvides patient-specific pharmacotherapy recommendations and drug information to the medical and nursing staffSupervises pharmacy technicians in activities related to medication dispensing, ordering and inventory management.Thorough documentation of all clinical interventionsExemplifies the Ernest Health mission, vision and values and acts in accordance with Ernest Health policies and proceduresUphold departmental, and institutional, policy as appropriate
