Travel Physical Therapist - $2,422 per week

Jackson Therapy Partners is seeking a travel Home Health Physical Therapist for a travel job in Lexington, Kentucky. Join as a traveling Physical Therapist providing home health care where you will help patients recover from injuries and manage chronic conditions while experiencing life in a new location. Minimum Requirements Associate's, Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA. Active Kentucky Physical Therapist license required to start the assignment. BLS certification required. Experience with OASIS documentation. Experience with HCHB or PointClickCare (PointCare) electronic documentation. Home health experience preferred. Minimum education requirement: Associate degree (client-specific). Assignment Details Primary specialty: Physical Therapist. Facility type: Home Health. Shift: Days. Shift hours: Monday–Friday 08:00–16:00 (client notes indicate Monday–Friday 08:00–16:30 EST schedule and Days 08:00–16:00). Weekend/on-call requirements: One weekend per month and one weekday on-call per week. Coverage areas: Madison, Montgomery, Clark, Powell counties. Typical responsibilities: SOC, evaluations, recertifications, ROC, agency discharges within scope of practice. Mileage reimbursed between visits from first patient to last patient. Floating to other facilities is not indicated. Follow facility assignment as directed.
