Salary $27.37 - $41.06 Hourly $56,931 - $85,396 Annually This position is a Pay Band C7 Posting Details The Family Health Bureau under the Department of Health's Public Health Division works to improve the health of women, children, and families in New Mexico through policy, surveillance, assurance, and direct services provided to reproductive age women, infants, children, adolescents and their families, including children and youth with special healthcare needs. The staff in the bureau work to bring federal funds to New Mexico and provide guidance and oversight of program services provided in the regional health offices. Why does the job exist? This position provides a safety net and assurance services for Children and Youth with Special Healthcare needs with special dietary and nutritional requirements for Children's Medical Services (CMS) and other programs within the Public Health Division (PHD). Services and concerns are addressed on both an individual/family level and on a systems level. Interviews are anticipated to be conducted with two weeks of certified list. How does it get done? Supports program operations by meeting caseload goals by performing clinic or facilities duties, traveling to clinics to perform services, outreach to health fairs and other programs to increase caseload, and provide customer service to applicants/participants. May provide intake/certification process of the applicant/participant including observation form, health assessment, breastfeeding women/infant evaluation, breast feeding women/infant/child midpoint assessments, anthropometrics (heights/weights/hemoglobin), and immunizations status by asking participant. Facilitate Nutrition Education classes using ¿Facilitators of Change¿- Participant Centered Services (PCS) design. May provide clinical nutrition assessments or prescribe food packages. Provides group and one-on-one nutrition education, including breastfeeding promotion and support that is responsive to the identified needs/interests of each participant. Identifies the need for individual care plans and implements them for low-risk participants. Refers participants to other health and social services and provides appropriate follow-up to referrals. Documents nutrition services provided, including referral and follow-up to referrals. Who are the customers? The public and Children's Medical Services Program participants. Ideal Candidate Fluent in both English and Spanish. Assisting CMS clients with linkages to community based nutrition service providers who work within systmes that limit access and support to ongoing nutrition care. Obesity prevention and the prevention of diabetes. Working within the Medicaid and insurance structures to assure children/youth with special healthcare needs nutritional/dietitic needs are met. Establishing community partnerships to work within systems and advocate for clients, access to nutrition services (screening, assessment, and on-going nutrition care). Minimum Qualification Bachelor's degree in a social, community, health, education, or nutritional field and two (2) years of experience as it pertains to the essential duties and responsibilities of the classification. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling six (6) years may substitute for the required education and experience. Employment Requirements Must possess and maintain a valid Driver's License. Working Conditions Position will provide services primarily in communities, clinics, and auxiliary sites, Work is performed in an office setting with exposure to Visual/Video Display Terminal (VDT) and extensive personal computer and phone usage with extended periods of office nose , bending, standing and lifting. Ability to lift up to 50 lbs. Services are provided in a clinic with diverse racial and ethnic population, many who do not speak the English language. Travel to satellite office to include overnight and weekend travel to attend meeting, training sessions and special events is required.
