Clinical Pharmacist for Medicare STARS - Remote

Overview Sentara Health Plans is hiring a Clinical Pharmacist for Medicare STARS – a full‑time, permanent position (40 hours per week). Standard working hours are 8am to 5pm EST, Monday‑Friday. Remote opportunities are available in the following states: Alabama Delaware Florida Georgia Idaho Indiana Kansas Louisiana Maine Maryland Massachusetts Minnesota Nebraska Nevada New Hampshire North Carolina North Dakota Ohio Oklahoma Pennsylvania South Carolina South Dakota Tennessee Texas Utah Virginia Washington (state) West Virginia Wisconsin Wyoming Responsibilities Demonstrate comprehensive knowledge of current pharmacotherapeutic principles, medical terminology, pathologies, disease states, population management, age‑appropriate therapy, and related medical or pharmaceutical care management. Work with Sentara Health Plan members to close medication‑related gaps in care, process referrals from the care team, counsel members on their medication therapy, and provide member education.
