Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: 5+ years of recent acute care case management experience in a hospital setting; no more than 6 months gap from last acute care case management role. License: Compact or state license in hand, no exceptions. Certifications: BLS required (must be via AHA); CCM or ACM preferred. Must-Have: Recent experience in an adult acute hospital setting. Experience with discharge planning. Level I, II, or III hospital experience for case management. Census familiarity with care management model. Competency in all care management functions, including CMS regulations. Epic experience required. No prior license or background hits. Description: Candidate must have experience in acute inpatient discharge planning. The role involves managing a fast-paced unit with multiple discharges and admissions daily. A strong understanding of UR (Utilization Review) is required. The position includes coordinating care for a patient population with varied needs. Demonstrated skills in appeals, denials, audits, and medical necessity determination are essential. Experience with CMS guidelines and concurrent reviews is necessary. Familiarity with HIPAA regulations is important. The candidate will be working with various interdisciplinary teams to support patient care. This position requires effective communication and coordination skills to ensure seamless discharge processes. Flexibility to work weekends and holidays may be needed.
