divh2Licensed Practical Nurse (LPN) - Clinic/h2pThe Licensed Practical Nurse (LPN) - Clinic provides support to providers in the delivery of timely, quality patient health care services in a clinic environment. Some duties may include collecting patient information and data, provide patient care, communicate with patient, and care team member. May work in a team supporting multiple providers. Individuals in this role will work in an outpatient clinic setting./ppPrinciple Responsibilities:/pulliProvides patient care support as delegated by Provider or RN ulliAdministers therapies including medications, scheduling follow up visits (and prior authorization) or other support unique to a specialty provider./liliProvides patient care pre/post encounter or procedure, preventative or other health information. RN./liliCollaborates with Provider and team on improving and managing quality goals./liliMay start, monitor, and discontinue IVs (dependent on site needs and training)./li/ul/liliRooms patients, manages patient flow and obtains adequate patient medical information/data. ulliPerforms rooming and accurately measures patients vital signs and other measurement/indicators as necessary and records in medical record./liliCoordinates communication of medical information and confirms information with patient when necessary and document per practice. policy and standards/liliAssists patients, as necessary, to prepare for physician exam, including providing psycho-emotional support./liliAssists in the collection, labeling and processing specimens timely/accurately./liliAssists physician with procedures including preparation of medication as required/needed./liliOrganizes patient flow so physician can move efficiently from one room to the next./liliProvides outbound calls to patients to follow up as directed which may include notifying patients of test results./liliAccurately and completely charts information in the patient medical record./liliMay checkout and schedule follow up visits with provider or appropriate staff before patient leaves./li/ul/liliReceives or initiates patient phone calls. ulliAnswers phones cordially and professionally in a timely manner./liliMay confirm or reiterate patient symptom using patients own words./liliArranges for appropriate appointment or forward to RN or provider for review/recommendation./liliDocuments all encounters and patient interactions accurately and completely in the electronic medical record./li/ul/liliProcesses physician orders. ulliPerforms any clinical functions within scope of license as ordered by physician./liliResponds promptly to patient needs and requests./liliReviews patient charts before the visit to ensure that test results, therapy reports etc. are available./liliProvides appropriate instructions to patient as directed by physician./li/ul/liliCleans, prepares, organizes and re-stocks exam rooms. ulliCleans and ensures the proper maintenance occurs for all exam rooms and sterilize instruments as trained./liliEnsures that required teaching supplies are restocked and readily available./liliStocks and supplies exam rooms according to standards and patient needs./li/ul/liliOther duties as assigned./li/ulpRequired Qualifications:/pulliAssociates or Vocational degree from a Board of Nursing approved LPN program/li/ulpPreferred Qualifications:/pulliTwo years clinic LPN experience/li/ulpLicenses/Certifications:/pulliLicensed Practical Nurse MN BON or Licensed Practical Nurse Wisconsin (if working in Wisconsin or interacting with Wisconsin patients remotely)/liliMay require both state licenses based on the services provided at the location hired/liliMust meet at least ONE of the requirements below:/liliCurrent BLS certification from the American Heart Association/liliCurrent BLS certification from the American Red Cross/liliAllina in-house BLS training (within 30 days of hire)/liliBLS is not required for 62000617 RN Support Resources In Basket and 62000629 Anticoagulation (INR) Program/li/ulpPhysical Demands:/pulliLight Work:/liliLifting weight up to 20 lbs. occasionally, up to 10 lbs. frequently/li/ulpPay Range:/ppPay Range: $24.62 to $33.74 per hour. The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable).
