Job Title: Clinical Pharmacist – Ambulatory Care

Location: JPS Primary Care Clinics, Texas
Schedule: Weekdays, 40 hours/week (4×10-hour shifts preferred)

Job Description:

Seeking an experienced Clinical Pharmacist to provide direct patient care across two JPS primary care clinics. Responsibilities include medication management, optimizing drug therapy, monitoring drug interactions and adverse effects, reducing polypharmacy, developing patient care plans, and collaborating with multidisciplinary healthcare teams. The pharmacist will also support clinical education, pharmacy safety initiatives, and formulary management.

Requirements:

• BCACP board certification
• PGY2 Ambulatory Care residency
• Minimum 4 years of direct patient care experience in medication management
• Active Texas State Board of Pharmacy license
• BLS certification
• Onsite patient care for approximately 16 patients per day
