BayCare Health Systems is currently seeking a Discharge Planner Pool to join our outstanding and compassionate team at North Bay Recovery Center ! Facility: North Bay Recovery Center Status: Pool, Hourly Shift Hours: 8:00 am-4:30 am On Call: No The Discharge Planner Pool responsibilities include: Provides assessment, linkage, monitoring, planning and advocacy for adult and or children experiencing acute mental health and or substance abuse issues in an acute care inpatient setting. Possesses a clear understanding of discharge and transition planning and linkage with community resources. Oversees legal status issues related to the individuals involuntary or in accordance with the Baker Act regulations. Provides support as active member of the treatment team. Will perform other duties as assigned by supervisor. Travel may be required. Required Certification: BLS (Basic Life Support) Required Experience: Behavioral Health - 1 year Child and Adolescent - 2 years (SIPP Only) Required Education: Bachelors - Counseling/Health Education/Related Field/Social Work/Human Services/Psychology/Nursing Master's in human services - Preferred Come join our wonderful team!
