Case Manager

Housing Forward

Housing Forward is seeking a dedicated Specialist to serve as the front door to the agency’s homelessness crisis-response. You will perform in-person assessments for walk-ins to determine immediate housing and stabilization needs and connect clients with case management, referrals, and financial or employment assistance. The role includes crisis-diversion duties, coordinating with multiple Housing Forward services, and maintaining HMIS/Salesforce records with timely notes. #J-18808-Ljbffr
