Clinical Supervisor RN

We are seeking a compassionate and experienced Hospice Clinical Supervisor, RN to support our clinical team at our Medina, Ohio office. In this leadership role, you will help coordinate patient care, support and develop clinical staff, and ensure the delivery of high-quality hospice services to patients and families. What You'll Do Coordinate and oversee direct and indirect patient care services provided by clinical personnel. Provide supervision, guidance, and support to clinical staff to promote quality care and compliance with established standards. Assist the Director of Clinical Services with clinical operations, staff development, and patient care oversight. Conduct medical record reviews and chart audits to ensure accurate documentation and quality care. Collect data and participate in Quality Assurance and Performance Improvement (QAPI) activities. Review initial intake screening information to help determine patient needs and the appropriateness of services before assigning direct care staff. Assist with onboarding and orientation of new clinical employees. Provide in-service education, coaching, and ongoing support to clinical team members. Conduct home visits and provide direct patient care as needed. Monitor medical supply needs and help ensure appropriate supplies are available for patient care. Support compliance with applicable Medicare requirements, Ohio regulations, and ACHC standards. Participate in performance evaluations and provide constructive feedback, education, and counseling to support employee development. Promote professional growth through ongoing education, training, workshops, and industry best practices. Perform additional clinical leadership responsibilities as assigned by the Director of Clinical Services. What We're Looking For Graduate of an accredited school of nursing. Current, unrestricted Registered Nurse (RN) license in the State of Ohio. Minimum of 2 years of nursing experience in home hospice or skilled home care ; hospice experience preferred. Previous clinical leadership or supervisory experience preferred. Knowledge of hospice clinical operations, Medicare requirements, applicable state regulations, and ACHC standards. Strong clinical assessment, organizational, and decision-making skills. Ability to support, mentor, and develop clinical team members. Excellent verbal and written communication skills. Professional demeanor and the ability to communicate effectively with patients, families, staff, and community partners. Positive attitude, self-confidence, and commitment to achieving organizational goals. Ability to work independently while collaborating effectively with the interdisciplinary team. Valid driver's license, appropriate automobile insurance, and reliable transportation. Work Environment & Physical Requirements This position involves a combination of office-based responsibilities and travel to patient homes or other care settings as needed. The role may require: Sitting, standing, bending, stooping, and reaching. Occasional lifting and carrying of medical supplies or records. Travel to patient homes to supervise care or provide clinical services. Accessing patient homes and care environments with varying physical accessibility. Sufficient hearing, vision, and physical ability, with reasonable accommodation where applicable, to perform essential job functions, including patient assessment and clinical care. Technology & Equipment The Hospice Clinical Supervisor must be comfortable using computers, electronic medical records, multi-line telephone systems, calculators, copiers, and other office or medical equipment necessary to perform the role.
