Certification Details BLS Massachusetts State Licensure Job Details MFM sonographer for busy department with 6 rooms Job Requirements Primary Source Verification of professional license and/or certification, as required for the position, within 30 days of start date and updated upon expiration. MA Conditional Licenses not accepted. Documentation of two measles vaccines, two mumps vaccines, and one rubella vaccine or two MMR vaccines, or proof of immunity via blood test. Vaccine required during flu season October 1, 2025 - March 15, 2026. No exemptions or waivers allowed. Must have documentation of TB skin test screening within 3 months of start date and updated if >6 month break, or documentation of a negative QuantiFERON TB Test or T-spot within 3 months of start date and updated if >6 month break. Physician Statement signed by MD/DO/NP/PA within 365 days of start date and updated if >6 month break. MGB Criminal Background Check within 45 days prior to start date and updated if >6 month break. MGB Massachusetts CORI background check within 30 days prior to start date and updated if >6 month break. Include past 7 years of work experience with city & state. 2 references within 4 years of submission, minimum 1 Supervisor or above in skillset. MGB Skills Checklist must be completed within 1 year of submission. MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions, upon acceptance of assignment. Verified highest level education achieved through background check. MGB SSN Trace must be run with Criminal check and list addresses/aliases for past 7 years. MGB National Sex Offender Check ran within 30 days of start and updated annually. MGB Exclusion Checks ran within 30 days of start and annually, including OIG, GSA, MassHealth & NH Medicaid exclusion lists. Unit Specific Information Maternal-Fetal Medicine (MFM) specialty Additional Details BLS certification required for onboarding (AHA, Red Cross, CHSF or Military). Fit Test (N95) within 365 days to start date and updated annually; no exemptions allowed. Hepatitis B vaccination strongly preferred. Tdap vaccine within 10 years preferred. Positive PPD requires negative chest x-ray documentation. Respirator Medical Clearance Attestation required if fit test is required. MGB TB Symptom Survey if Positive PPD recommended.
