Clinical Case ManagerThe Clinical Case Manager provides case management, outpatient clinical services, and therapeutic support across all Innova Recovery Center locations. This role coordinates client care throughout the treatment continuum, assists clients in accessing needed resources, and promotes successful engagement in treatment and recovery. Responsibilities include coordinating care, developing discharge plans, maintaining an outpatient caseload, facilitating therapeutic groups, and collaborating with multidisciplinary treatment teams to ensure continuity of care and positive client outcomes.Job Duties and Responsibilities:Provides clinical case management services across all Innova Recovery Center locations.Coordinates the plan of care for clients in Residential (RTC), Partial Hospitalization (PHP), Intensive Outpatient (IOP), and Outpatient (OP) programs.Maintains an outpatient caseload, providing individual therapy sessions as assigned.Facilitates therapeutic and psychoeducational groups across levels of care as assigned.Assists with client assessments, treatment planning, and monitoring client progress.Participates in multidisciplinary treatment team meetings, clinical supervision, and leadership meetings.Serves as a liaison between clients, families, referral sources, and outside providers.Connects clients with community resources and coordinates referrals.Schedules and coordinates medical, psychiatric, and other client appointments.Develops and coordinates discharge plans and aftercare services.Assists clients and families with understanding discharge recommendations and available resources.Coordinates ongoing client care, including outpatient services, medication management, and community providers.Meet clients and/or guardians regularly to assess needs and provide case management support.Communicates with the Utilization Review team regarding level of care, authorizations, and discharge planning.Coordinates with outside providers to support continuity of care.Completes documentation related to admissions, transfers, discharges, case management services, therapy sessions, and group facilitation.Performs discharge planning audits to ensure compliance with organizational standards.Builds and maintains relationships with community providers and referral sources.Assists with community outreach, networking, and marketing efforts as needed.Coordinates with Residential, PHP, IOP, and Outpatient teams to ensure continuity of care.Performs other duties as assigned.Minimum Qualifications:Bachelor's degree in social work, Psychology, Counseling, Human Services, or a related behavioral health field required.LMSW or other behavioral health licensure preferred.Minimum of three (3) years of clinical case management experience.Minimum of three (3) years of experience working in a behavioral health, mental health, or substance use treatment setting.Experience coordinating care across multiple levels of treatment preferred.Experience facilitating therapeutic or psychoeducational groups preferred.Experience maintaining an outpatient caseload and providing individual counseling preferred.Strong working knowledge of Electronic Medical Records (EMR) systems and Microsoft Office Suite.Valid driver's license and ability to travel between Innova Recovery Center locations as required.Competencies:Knowledge of behavioral healthcare services and the continuum of care.Understanding case management principles, discharge planning, and community resource coordination.Ability to facilitate therapeutic and psychoeducational groups.Ability to build rapport and maintain therapeutic relationships with clients and families.Strong organizational, time management, and problem-solving skills.Ability to coordinate care with multidisciplinary teams and outside providers.Effective verbal and written communication skills.Ability to maintain accurate, timely, and compliant clinical documentation.Ability to prioritize multiple responsibilities while maintaining quality of care.Ability to exercise sound judgment, maintain confidentiality, and uphold professional boundaries. #J-18808-Ljbffr Priority Dispatch Corp.
