LPN Home Health & Hospice Location: Tulsa, OK | Contract: 13-week (high possibility of extension) | Start: 1/20/2026 | Hours: 40 hrs/week | Shift: Mon-Fri 8a-4:30p | No Call Pay: $40.00/hr - $48.00/hr | Travel: $1,950/week Key Responsibilities Provide skilled nursing care to patients in their homes under the supervision of an RN and in accordance with the care plan. Administer medications, treatments, and wound care while monitoring patient responses and reporting changes in condition. Educate patients and families on disease management, medication administration, and self-care techniques. Document all care, observations, and interventions accurately and timely per agency and regulatory requirements. Collaborate with physicians, RNs, and other healthcare professionals to ensure safe, effective, and coordinated patient care.
