Location: Richmond, CA, with travel throughout Contra Costa and Alameda County
Pay: $22/hr
End date:  March 31, 2027

Our History
Brighter Beginnings is a 501c3 nonprofit organization and has been responding to the needs of families in resource-poor neighborhoods since 1984 when our work began in response to the alarming differences in mortality and illness rates among African American babies compared to other children.

Brighter Beginnings has grown into a respected and well-connected organization with locations in Oakland, Richmond, Bay Point, and Antioch. We have a multi-cultural, bi-lingual staff of passionate and committed family service professionals; in fact, many of our staff came to this work because of the challenges they faced in their own families.


Position Summary
This position is funded through a grant administered by Essential Access and supported by the federal Title X Family Planning Program, scheduled for April 1, 2026 – March 31, 2027. The primary responsibility of this role is to support and ensure compliance with all grant requirements, funding guidelines, reporting standards, required educational activities, and applicable regulations. The position supports the implementation and oversight of grant-funded activities through program monitoring, documentation, data collection, quality assurance efforts, and assistance with program reporting.

Serving as a Community Health Worker and community liaison, this role connects patients with health, social, and supportive resources while helping to reduce barriers to care. Responsibilities include providing sexual and reproductive health education, outreach, resource navigation, patient scheduling, appointment follow-up, and documentation within the electronic health record (EHR). The incumbent works collaboratively with patients, healthcare providers, community organizations, and external agencies to coordinate services, facilitate referrals, and improve access to care.

The position also supports community engagement and outreach activities to promote program services, increase awareness of available resources, and foster partnerships that advance community health. Through collaboration with staff, community partners, and stakeholders, this role helps ensure program goals are achieved, grant requirements are met, and equitable access to high-quality family planning and reproductive health services is maintained.

 

QUALIFICATIONS:

  1. Lived experience that fosters a meaningful connection to the population served. This may include, but is not limited to, lived experience related to incarceration, military service, pregnancy and childbirth, disability, foster care involvement, homelessness, mental health conditions, substance use, or being a survivor of domestic or intimate partner violence, abuse, or exploitation. Lived experience may also include shared race, ethnicity, language, sexual orientation, gender identity, cultural background, or other characteristics reflective of the communities served.
  2. Experience in community education, outreach, health promotion, resource navigation, social service linkage, or advocacy for community health and social service needs.
  3. Excellent interpersonal, communication, and relationship-building skills, with the ability to engage and work effectively with individuals from diverse cultural, linguistic, and socioeconomic backgrounds.
  4. Strong organizational, time management, and computer skills, including proficiency with electronic documentation, data entry, and standard office software.
  5. Ability to work independently while maintaining a cooperative and collaborative approach within a multidisciplinary team environment.
  6. Ability to perform in-person outreach activities, including setting up materials for community events, standing for extended periods, and walking for sustained periods during outreach and canvassing activities.
  7. Possession of a valid California Driver’s License, current automobile insurance, and access to a reliable personal vehicle for community-based work and travel.

Preferred Qualifications

  • Bilingual proficiency in English and Spanish or another language commonly spoken within the communities served.
  • Associate’s degree or higher in Public Health, Sociology, Nursing, Social Work, Health Education, or a related field.
 

RESPONSIBILITIES:

Primary Job Functions

  1. Provide services in accordance with Essential Access guidelines
  2. Participate in community engagement and outreach activities to promote Brighter Beginnings services and provide information to community members.
  3. Collaborate with community organizations, attend partnership and collaborative meetings, and support community groups in health-related activities.
  4. Participate in required staff meetings, trainings, and other collaborative or program-related activities.
  5. Maintain accurate, timely, and complete documentation of all client interactions in accordance with Brighter Beginnings policies and procedures.
  6. Conduct patient outreach, including follow-up calls to check in with patients and support appointment scheduling and attendance.
  7. Enter and maintain patient information, including social determinants of health, in the electronic health record (EHR).
  8. Provide and share health education, prevention information, and health promotion resources and trainings.
  9. Support the planning, organization, and coordination of Family Planning Program activities to ensure goals and objectives are met within required timelines and funding parameters.
  10. Utilize database and information systems to support quality assurance activities, identify program trends, and assist with performance monitoring.
  11. Collaborate with program staff to assess client needs and improve service delivery and linkage to existing community resources.
  12. Assist with program reporting, including data collection and documentation required for grant compliance.
  13. Travel throughout Contra Costa and Alameda Counties as required to perform job duties and attend meetings or outreach activities.
  14. May be required to work occasional evenings and weekends based on program and community needs.

  3. Limited physical effort is required.
  4. Both standing and sitting are required, with most of the job time spent sitting. Approximately three-quarters of the time is spent using a computer keyboard.
  5. Various types of equipment/supplies are used to accomplish the job requirements and include, but are not limited to pens, pencils, calculators, computer keyboards, telephones, printers, etc.
  6. Required to drive to other work sites for meetings, conferences, etc.
