Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.\n MINIMUM REQUIRED QUALIFICATIONS –\n \n \n TX/compact RN license\n BLS\n 2 years case management experience in an acute hospital setting\n Acute hospital experience during the past year with no more than a 90-day break\n \n \n LENGTH OF ASSIGNMENT – 13 weeks\n SHIFT / HOURS PER WEEK – 7:00AM - 7:00PM Sat-Mon\n START DATE – ASAP\n \n Facility Location\n \n Known to locals as simply “Big D,” everything in this sprawling Texas town is larger than life.
