Early Intervention Service Coordinator
The Early Intervention Service Coordinator supports the System Point of Enter (SPOE) team by performing professional service coordination to families of infants and toddlers within the family's home, in a manner that reflects the Center for Human Services' (CHS) mission, vision, and values, while ensuring compliance with all State contract requirements.
Essential duties and responsibilities include:
- Providing intake and ongoing service coordination to families and children, serving as the Individual Family Service Plan (IFSP) team leader and facilitator of the IFSP process
- Demonstrating respect by taking into consideration the capabilities, backgrounds, and cultural differences of families, children, co-workers, and the public consistent with trauma informed practices
- Promoting the family capacity model of service delivery by monitoring the delivery of services to ensure that services are provided timely and in accordance with the IFSP and assisting families with referrals to available community resources in the coverage area for identified needs
- Participating in activities such as child find, public awareness, recruitment activities, and including active engagement as an Early Intervention Team (EIT) member
- Working independently from a home office with minimal supervision by maintaining accurate, complete, and timely electronic and paper file records, as mandated by the DESE First Steps contract
- Creating team unity by fulfilling and carrying out other duties as assigned by the Early Intervention Director that support the SPOE contract requirements and timelines
Required education includes a Bachelor's Degree (BA, BS) in one of the following with one year of documented experience working with families and children; Early Childhood Special Education, Early Childhood, Elementary Education, Special Education, Child/Human Development, Social Work, Nursing, Psychology, Education Administration, Sociology, Family Science/Studies, Counseling OR a Bachelor's Degree (BA, BS) or higher with three years of documented experience working with families and young children.
Required qualifications include:
- Must have a valid driver's license
- Must provide proof of personal valid vehicle insurance
- Background screenings required prior to start
- Fingerprinting after starting
Preferred experience and considerations include:
- Knowledge of Paycom platform, WebSPOE preferred
- It is preferred that the Service Coordinator live in the primary service area

Physical requirements include:
- Prolonged periods sitting at a desk, working on a computer, and traveling in a vehicle
- Must be able to lift 10 pounds at times and occasionally lift and/or move up to 25 pounds
