Quality Support Services (QSS) LPN or RN

Join RKM and make a difference in the community by providing primary care in an ambulatory health setting as part of a primary care team. Since 1999, our mission is to provide open access to quality patient centered healthcare in the communities in which we live. RKM Primary Care is a Federally Qualified Health Center with 11 clinics in 5 parishes.

Job Duties:

• Triage Coordinate with providers and nurses to resolve health concerns.
• Focus on assessments.
• Integrated Care Promote the delivery of care across the continuum of services.
• Care Gap attainment of identified preventive and chronic disease gaps in care.
• Failed Appointments Follow-up on cancelled/no show appointments.
• Lab tracking/Medical - Monitor lab orders.
• Data - Prepare data on Referrals.

Qualifications/Skills:

• Current RN or LPN license in good standing with the State of Louisiana (Required).
• Current Basic Life Support (BLS) (Required).
• Prefer experience in ambulatory care setting.

Employment Type: Full Time, Monday through Friday

Starting Salary Range :

• LPN - $18.66 – $24.35 per hour; up to 3% annual increases.
• RN - $50,230 - $71,240 per year; up to 3% annual increases.
