Active Washington state PT license required prior to start of assignment. BLS/CPR certification required (American Heart Association or equivalent). Home health experience required. Experience with SOC, evaluations, ROC, recertifications, and subsequent visits preferred. Familiarity with home health electronic medical records preferred. Previous use of HCHB EMR is a plus. Ability to travel within a 50 mile radius for patient visits. Business casual dress expected. Assignment Details Facility Type: Home Health. Shift Name: Days. Shift Details: Days 0800-1600. Weekly Hours: 40 hours per week guaranteed. On Call/Weekend Requirement: No. Coverage Area: Up to 50 mile radius from base location. Primary Responsibilities: Start of Care (SOC), evaluations, recertifications, return-to-care (ROC), and follow-up visits.
