Job Description

The Clinical Resource Unit is committed to a Nursing Professional Practice Model. We believe in caring relationships with patients, families, community, and colleagues to provide exceptional care. Our nursing staff, nurse leaders, interdisciplinary team members, and physician partners provide specialized medical and nursing care to various inpatient units. Diverse medical diagnoses include coronary artery disease, myocardial infarction, strokes, cancer, cellulitis, post-surgical care, and more. Job Responsibilities

• Perform all responsibilities and duties required by the unit as specific to the scope of service.
• Implement procedures and theories related to the specific area of practice.
• Maintain accurate documentation of nursing care provided to ensure integration of information for the healthcare team.
• Complete initial assessments upon patient admission and develop appropriate plans of care that are based on patient and family needs.
• Implement plans of care, nursing interventions, and patient care procedures.
• Modify plans of care based upon continuous evaluation.
• Report and/or correct safety hazards and incidents to ensure safety for patients and employees.
• Communicate with others directly and privately to resolve issues when necessary.
• Offer constructive feedback to assist in the professional development of peers.
• Confer with colleagues to expand knowledge base.
• Identify own learning needs and take initiative to begin appropriate solutions.
• Fulfill departmental competency requirements.
• Float to various inpatient units as needed.
• Provide direct nursing care and collaborate with multidisciplinary team members to deliver patient care.
• Conduct and document assessments, nursing diagnoses, plans, implementations, and evaluations of patient care.
• Act as a resource to staff from various units.
• Coordinate care with multidisciplinary team members to facilitate high-quality patient care.
• Manage care of acute and chronically ill cardiac and pulmonary patients.
• Collaboratively identify patient goals of care and design care plans with patients and families.
• Implement necessary interventions including medication administration and evaluation.
• Prepare patients for procedures, provide pain management, and monitor labs.
• Discharge patients and provide instructions for safe transition from hospital.
• Document all care using EMR.
• Ensure team huddles occur and attend them.
• Delegate tasks with appropriate follow-up.
• Answer call lights promptly.
• Ensure basic care is provided according to standards.
• Handle conflict management while maintaining professional communication.
• Provide post-operative care for complex surgical populations.
