RN - Home Health/Hospice

Title: RN - Home Health/Hospice
Duration: 3 months contract + Possibility of Extension
Location: Corpus Christi, TX
Coverage Areas: 60 mile radius from office
Shift: M-F 8 AM - 6 PM
back up on call one evening/night during the week and a weekend back up on call every 4-6 weeks

Roles & Responsibilities:
Manage a caseload of hospice patients across home and facility-based settings.
Develop, implement, monitor, and update individualized plans of care in collaboration with physicians and the interdisciplinary team.
Coordinate services to ensure continuity of care and timely response to patient needs.
Conduct comprehensive nursing assessments and ongoing patient evaluations.
Educate patients and caregivers regarding:
Hospice philosophy and services
Disease progression
Symptom management
Medication administration
End-of-life care planning
Provide emotional support and compassionate guidance to patients and families.
Advocate for patient goals, preferences, and care decisions.

Minimum Qualifications:
Active and unrestricted Registered Nurse (RN) License
Valid driver's license and reliable transportation
Current automobile insurance
Ability to travel throughout the assigned service territory

Preferred Qualifications:
Previous Hospice Nursing Experience
Previous RN Case Management Experience
Travel nursing experience
Experience providing care in homes, assisted living facilities, and skilled nursing facilities
Strong knowledge of hospice regulations, symptom management, and end-of-life care practices
