Growing together.Optum Infusion Pharmacy has an immediate opening for a friendly, patient focused and detail-oriented Patient Care Coordinator to join our team.The primary role of the Patient Care Coordinator on the National Start of Care Nursing Team is to provide nursing coordination for infusion services for all Optum Infusion Pharmacy patients, coordinating the daily activities of referral, intake, staff/agency assignment and patient management. The Patient Care Coordinator works cooperatively with all members of the care team to support the vision and mission of the organization and deliver excellent customer service.Shift Schedule (up to 10 hours per week): Saturdays 8:30am - 5pm CST (may be asked to cover shifts during week if available based on business needs)You'll enjoy the flexibility to work remotely * from anywhere within the U.S. as you take on some tough challenges.Primary Responsibilities:Receives and executes the initial patient intake process to assure that patient verification and ongoing scheduling of nursing services occur in a timely mannerAssures timely communication with other company personnel, and outside providers, especially Optum nursing and other agencies, to assure the quality and continuity of patient careAnswer telephone calls promptly and courteouslyAbility to establish and maintain positive clinical rapport with all patients and their families, representing Optum Infusion Pharmacy including working directly with patients and their families, maintain contact as per company policy and protocolCommunicates coordination of care with alternate providers (AIS, hospice) and documents communicationCompletes all required patient care documentation and reports accurately and in a timely manner, according to internal and external regulatory requirementsTracks schedule of visitsAssists with staffing patient visits to subcontracted home health agencies, including sending/retrieving all required documentation for patient managementCollaborative relationships involving agencies delivering services on behalf of/or in conjunction with Optum Infusion Pharmacy, including nursing agencies in a contracted relationship, patient, families, significant caregivers, and other healthcare team membersVerifies appropriate nursing visit authorization is obtainedVerifies patient demographics during the visit scheduling processParticipates in staff meetings with nursing and pharmacyMaintain all mandatory education required upon hire and annually per policyAlways represents self in a professional and ethical manner, while representing Optum Infusion Pharmacy, complying with all applicable company policies and proceduresPerform other related duties and assignments as requiredYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear directions on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:6+ months of customer service or healthcare related experienceIntermediate level of computer proficiency including Microsoft Office Word, Excel and Outlook and multiple web applicationsMust be 18 years of age OR olderPreferred Qualifications:1+ years of experience working in a front office medical position performing duties such as scheduling appointments, checking patients in/out and maintaining medical recordsKnowledge of EMR softwareSoft Skills:Ability to work independently and as a team, and maintain good judgment and accountabilityAbility to multi-task and prioritize tasks to meet all deadlinesAbility to work well under pressure in a fast-paced environmentDemonstrated ability to work well with health care providersStrong organizational and time management skillsExcellent verbal and written communication skills; ability to speak clearly and concisely, conveying information in a manner that others can understand, as well as ability to understand and interpret information from othersPay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $18 - $32 per hour based on full-time employment.
